Biggest mistake I ever made was moving a stop on $EURUSD during a volatile NFP release. Had a tight stop, price spiked, and instead of taking the small loss, I shifted it 'just a bit' thinking it would retrace. It didn't. Watched a manageable paper loss turn into a significant drawdown that wiped out a month of consistent gains. Took me a while to recover psychologically from that one.
