Confused about how to properly journal trades, especially with partial exits?

Hey everyone, fairly new to actively trading crypto beyond just holding. I'm trying to get into the habit of journaling my trades to learn from them, but I'm a bit lost on the best way to record partial exits. If I enter a position, then scale out at a couple of different price points, do you record each exit as a separate 'trade' or as part of the initial entry? It feels like treating them separately skews my win/loss ratio, but combining them makes it hard to see the performance of each exit strategy. How do you guys manage this in your journals, especially for something like $ETH where volatility can lead to multiple scaling opportunities?
