Confused about how to properly journal my trades

I've been trying to keep a trade journal for my $EURUSD trades, noting entry, exit, R:R, and a quick thought, but I'm not sure if I'm capturing the right details or if there's a more effective way to analyze what went wrong or right; what do you guys focus on most?

    It sounds like you've got the main bases covered already. For me, beyond the entry/exit, I really focus on my emotional state before and during the trade. Sometimes that's the biggest factor in what goes wrong or right.
